The problem arrives quietly. An AI agent gets stuck, looping on the same task like a broken record. Or it invents facts with unsettling confidence. It fumbles the tools it's supposed to use, confuses customers, and drives them away—all while the engineering dashboard shows green lights across the board. Everything looks fine. Technically, it is fine.

Except your conversion rate is tanking.

By the time someone notices, the damage is done. Dozens of users, maybe hundreds, have already left. This is the maddening reality of deploying AI agents in production: the failures that matter most are the ones traditional monitoring tools never catch.

Enter Sentrial, a startup out of Y Combinator's Winter 2026 batch. The San Francisco outfit promises something that sounds almost too useful to be true—real-time detection of agent failures, followed by root-cause diagnosis and specific code fixes. Within a month of its mid-February launch, the company had already signed 15 paying customers, a traction that suggests the founders aren't the only ones losing sleep over invisible agent meltdowns.

The Blind Spot in Production AI

Neel Sharma and Anay Shukla, both UC Berkeley computer science graduates, built Sentrial after living through the frustration themselves. Sharma worked on agentic optimization at Sense; Shukla deployed agents at Accenture. They kept running into the same wall: infrastructure monitoring tells you when servers crash or APIs time out, but it stays silent when your AI quietly destroys user trust.

So they built something different. Sentrial watches production conversations as they unfold, analyzing patterns in dialogue flow, model outputs, and tool interactions. It flags loops, hallucinations, tool misuse, user frustration—the behavioral breakdowns that slip past conventional metrics.

The onboarding is deliberately frictionless. Engineers can instrument their code in roughly five minutes using SDK integrations with LangChain, CrewAI, AutoGen, Claude Code, Vercel AI SDK, and Mastra. That speed matters when you're trying to convince skeptical teams to adopt yet another monitoring layer.

From Alert to Fix in One Click

Where Sentrial moves beyond standard observability is in closing the loop. Most monitoring platforms stop at notification—they tell you something's wrong, then leave you to dig through logs and trace routes. Sentrial goes further.

When it identifies an issue—say, an agent repeatedly calling the wrong tool—it connects to GitHub, indexes the relevant code, and surfaces an AI-powered chat interface loaded with issue and code context. Engineers can review suggested diffs and create a pull request with a single click. The GitHub integration uses minimal OAuth permissions and, according to the company, processes code in real-time without storing it on Sentrial's servers. (Whether that architecture holds up under regulatory scrutiny remains to be seen, but it's the right promise to make.)

The Python SDK, which hit version 0.6.0 on March 6, handles automatic session tracking for LLM calls, tool calls, and multi-turn conversations. It calculates token usage and cost, supports streaming, and offers client-side PII redaction before data ever leaves a developer's server. TypeScript support is available through integrations with frameworks like Mastra, Claude Code, and Vercel AI SDK.

Sentrial also tracks OpenTelemetry traces, situating itself within an increasingly crowded ecosystem of agent observability tools—LangSmith, Langfuse, Arize Phoenix, Traceloop. What sets it apart, at least on paper, is the emphasis on behavioral detection and actionable fixes rather than just metrics dashboards.

Testing in Production, Without the Terror

On March 20, Sentrial announced an "Experiments" feature that lets teams test prompt changes on live traffic and measure impact across metrics like user frustration, refusals, accuracy, and what they're calling "forgetfulness." It's a shift from reactive debugging toward proactive iteration—a hint that the founders see their platform as something more ambitious than a monitoring add-on.

The customer acquisition pace is notable, perhaps more than the founders expected this early. Sharma reported the 15th paying customer in a LinkedIn post dated roughly mid-March. The company hasn't disclosed customer names, pricing tiers, or public case studies yet. Documentation directs interested users to book a demo rather than sign up self-service—a choice that suggests either limited capacity or a deliberate focus on higher-touch enterprise deals.

The broader timing looks favorable, if competitive. AWS has been pushing observability for its Bedrock AgentCore runtime. LangChain's LangSmith positions production monitoring as essential infrastructure for moving agents out of the lab. Academic research from recent months—papers like AgentSight, Trajectory Guard, and AgentTrace—points to mounting concern around runtime safety and anomaly detection for agentic systems. Cisco expanded its AI Defense platform to include multi-turn agent testing. The market is waking up to a problem that, until recently, most teams assumed they could handle with existing tools.

They couldn't.

The Hard Part Still Ahead

Sentrial is very early. Two founders, a public launch measured in weeks, and a product that's still proving itself in production environments where the stakes are real and the users unforgiving. The documentation is live, the integrations are shipping, and the early traction suggests they've found a nerve.

But building a monitoring tool that developers actually trust—that doesn't cry wolf, that genuinely speeds up debugging instead of adding noise—is a different challenge than signing the first 15 customers. Whether the diagnosis-to-fix workflow remains genuinely useful at scale, or whether it becomes just another dashboard layer that teams learn to ignore, will depend on execution over the next several quarters.

For now, though, the value proposition is sharp enough: if your AI agent is quietly failing in production and you don't know why, Sentrial wants to tell you. And then help you fix it before the next hundred users walk away.

That's a promise worth watching.
