The receptionist at a busy rheumatology clinic doesn't just schedule appointments. She navigates a labyrinth: more than 70 different online portals, faxes that may or may not arrive, and phone trees that seem designed to test human patience. All to secure prior authorization for biologic medications that can cost tens of thousands of dollars annually per patient. For smaller infusion centers, the sheer weight of paperwork sometimes determines which patients they can afford to treat.

Enter Ruma Care—a startup so early-stage it's still just its two founders, both fresh from Y Combinator's Winter 2026 cohort. Meng Fei Shen and Christina Huang are building their AI-powered platform, and the pitch is audacious: collapse that entire administrative nightmare into a single automated system. Clinics in California and Nevada are already testing it, and if the early numbers hold, denial rates are falling by more than half while staff time per authorization shrinks to a fraction of what it was.

Whether a two-person operation can scale that promise into a sustainable business is another question entirely.

How It Works

Ruma Care bills itself as "the operations stack for biologic infusion clinics"—a phrase that sounds bland until you understand what it replaces. The platform connects directly to electronic health records, pulling patient, provider, and insurance data in real time. Then it does something deceptively complex: it figures out which submission criteria each insurer demands based on the medication, the diagnosis, and the specific plan. Different forms for different payers, different portals for different drugs. The system selects the correct documentation, fills it out, and fires it off—portal, fax, or paper, whatever the insurer prefers.

Prior authorizations are just the start. Ruma also auto-enrolls patients in manufacturer copay assistance programs and monitors reimbursement flows to catch potential denials before they snowball. On January 21, the founders released a free tool called the Ruma Formulary Navigator, which lets providers type in a plan, medication, and diagnosis to instantly see coverage rules and PA requirements, complete with citations from official payer documents.

The company focuses on rheumatology and adjacent specialties where biologic infusions are common—and where, according to Ruma's claims, insurers deny coverage roughly 37% of the time each year. That's a lot of hours spent resubmitting the same information in slightly different formats.

The Early Results (Such As They Are)

Clinics piloting the platform report denial rates dropping by 50% to 55%, with staff time per prior authorization falling by a factor of 20. To be clear: the company only launched in late January, so these figures reflect just a few weeks of real-world use. Promising, certainly. Definitive? Not yet.

One testimonial posted on the company's site comes from Janeen Thompson at San Mateo Rheumatology: "Ruma Care helps us get our prior authorizations done so fast, we have time to play." That's the kind of quote that sounds almost too good, but in healthcare administration, even modest improvements can feel transformative.

Ruma also co-hosted a Biologics Access Symposium with Stanford School of Medicine on January 24—a savvy move for a startup this young. Academic partnerships signal credibility, and in a sector as risk-averse as healthcare, credibility matters.

Who's Behind It

Shen and Huang met at Yale, where they both studied computer science. Before Ruma, they collaborated on consumer apps, but their healthcare credentials run deeper. Shen helped scale Uber One to more than 13 million members and later worked on AI integrations for over 100,000 suppliers at Walmart Data Ventures. Huang led integrations with UnitedHealthcare during a stint at Medallion and built robots, multimodal systems, and voice interfaces at Apple. According to their Y Combinator profile, both have worked on products for UnitedHealthcare and the U.S. Department of Health and Human Services—experience that presumably helps when you're trying to crack the Kafkaesque logic of insurance workflows.

Y Combinator backed the company in December 2025. A CB Insights profile lists a $500,000 convertible note, though Ruma hasn't publicly confirmed any funding beyond the YC batch investment. For now, the team remains just the two founders.

A Suddenly Crowded Field

Timing, as they say, is everything—and Ruma Care is arriving at a moment when the prior authorization space has become almost fashionably crowded. SamaCare launched "SamaCare Plus," an AI-driven touchless PA product, in November 2025, claiming it was trained on a dataset of two million prior authorizations. Plenful unveiled its own AI suite for intake and prior auth in September 2025, promising a 75% reduction in administrative work and signing customers like BioMatrix Specialty Infusion Pharmacy and Renown Health. Humata Health, which raised $25 million in mid-2024, announced a Renown Health partnership this past February.

Even the incumbents are waking up. In January, Availity and Abridge announced a collaboration to enable "real-time prior authorization" using ambient clinical AI—the kind of ambient listening tech that transcribes doctor-patient conversations and generates documentation on the fly. Optum, UnitedHealth's tech arm, published a corporate update in February highlighting its own AI-powered PA initiatives. The company noted that providers and administrative staff spend an average of 13 hours per week wrestling with prior authorizations, a statistic that sounds low to anyone who's actually worked in a clinic.

Then there's the regulatory push. CMS finalized a rule in January 2024 mandating faster response times and FHIR-based Prior Authorization APIs by January 2027. Major insurers—Blue Cross Blue Shield Association, Humana, and others—have pledged to eliminate or streamline prior authorizations, with goals for real-time electronic decisions by 2027. In December 2025, AHIP, the health insurance lobby, announced that by 2027, at least 80% of electronic PA approvals with complete documentation would be answered in real time.

Surescripts, the health information network, reported 30.5 billion transactions in 2025 in its March 2026 impact report—a 12.3% jump year-over-year. The industry is shifting toward interoperability standards that include electronic prior authorization, which means the underlying infrastructure is finally catching up to the problem.

The Open Question

For Ruma Care, the challenge isn't proving the technology works. Early clinic results suggest it does, at least in limited deployment. The challenge is scaling beyond California and Nevada while defending a narrow niche against well-capitalized competitors and incumbents that have spent years building their own tools. A two-person team has agility, but it also has limits—on bandwidth, on capital, on the sheer number of insurance relationships you can cultivate at once.

The prior authorization bottleneck has frustrated patients, providers, and even insurers for decades. Now everyone's trying to solve it at the same time. Whether Ruma can carve out a defensible slice of that market may come down to execution speed and how quickly the founders can turn pilot programs into long-term contracts. The window for a small startup to establish itself in healthcare infrastructure is rarely open for long.
